Eligibility Requirements

You must be:

1. A Kentucky Resident
2. At our clinic, we treat patients aged 18 and older
3. You must not have a Felony Disqualification

Must be diagnosed with one of the following qualifying medical conditions:

1. Any type or form of cancer;
2. Chronic or severe pain;
3. Epilepsy or other intractable seizure disorder;
4. Multiple sclerosis, muscle spasms, or spasticity;
5. Chronic nausea or cyclical vomiting syndrome; or
6. Post-traumatic stress disorder.
