2. Submit Your Medical Documentation
Once you have submitted a request in the above link in step 1, please also send us your SSN and a copy of your Driver's License or ID card with your address and date of birth included along with your medical records to housecalls@echp2u.com or by faxing them to 888-232-1268. This ensures your documentation includes a qualifying diagnosis from your practitioner.
3. See one of our Nurse Practitioners
A face-to-face assessment will be conducted, and our practitioner will review your medical records.
4. Receive Certification
If you qualify, we will submit your written Kentucky Medical Certification the same day.
5. Apply to the Cabinet
Then you must submit your own application, including the $25 fee. Click here to apply.
6. Receive Your ID Card
Await the Cabinet's decision and download your medical cannabis registry ID card once available, the cabinet decision can take up to 30 days.
